You can request refills of your prescriptions by one of the following methods:

1. Call (870) 932-1198 or toll free (888) 798-0700
2. Press 2 and then the # key
3. Be prepared to leave the following information: Caller’s Name, Callback number, Patient’s Name, Patient’s Date of Birth, Medications to be refilled, and the Pharmacy you would like to use.

Patient Portal 

1. You can now request prescription refills via our patient portal.
2. CLICK HERE to access the portal.
3. You wil then be asked to login or create an account.
